Clinical observations of surgical jaundice

Abstract
Clinical observation was made 109 patients of surgical jaundice who received surgical inter-vention at Chonnam University Hospital for 4 years from 1970 to 1974.
1) In 66 fl. of the case, jaundice was due to benign lesion and in the remaining 34% due to malignant lesion. It yielded common bile duct stone in 51.4% of the benign lesions.
.2) The seasonal distribution indicated frequent in October.
3) The sex ratio was 1.32 male to 1 female in benign groups and 1.5 male to 1 female in malignant group. The lage distribution indicated predominant incidence in 4th decade.
4) The duration of illness was as follows; with in one month, 59% in the benign groups and between 1 month to 6 months 41% in the malignant groups
5) The history of previous other disease or operation was obtained 41% of total case.
6) The predominant symptom was pain in 93% of benign group, and in 62% of malignant group. The tenderness was compained in 75% of benign group, and 48.6% of malignant, lesion.
7) In almost all the cases, the direct reacting bilirubin was markedly increased. This was especially true of malignant obstructions. The average of total serum bilirubin was 13.3% in malignant group. Alkaline phosphatase and S.G.O.T. were elevated more markedly in the malignant groups.
8) Various opreative procedure were performed in the cases; Choecystectomy with T-tube drainage in 27 case, cholecystectomy in 18 cases, by pass procedure in 19 cases, exploratory 10 cases, pancreaticoduodenectomy in 3 cases, other¢¥ in remainder,
9) Postoperative complication were in 42 cases (38%), infection being most common (18 cases), hemobilia 4 cases, residual stone 4 cases.
10) The mortality rate was in 9.2% of the total cases, 9.7% in the benign groups and 8.1% in malignant groups.
